Obviously it can’t be Eric Dane from Grey’s for two very obvious reasons:
He is not the show’s lead actor
& He is not getting fatter but skinnier.
I’d say Julian McMahon on Nip/Tuck. He’s by no means fat, but he’s starting to get a little pudgy and it’s kind of jarring considering what a vain womanizer his character is supposed to be.
Only problem is the show’s no in production, so I’m guessing producer, at the moment don’t care.

The only person who is in need of an intervention would be Hurley – but he’s probably very aware of the situation. Unless they are willing to grant him a great storyline which guarantees his spot in the series but explains his absence for awhile, and agree to pay for a lap band or something like that, I don’t know that the people I work with would be the right people to stage an intervention for me.
Whomever it is – all the actors mentioned in the comments are much loved by their numerous fans in whatever shape and size they happen to be.
